Windows Xp:
    Click Start, then click Run.
    In the Open box, type explorer and then click OK.
    Navigate to the Windows directory (e.g. a typical path may be C:\Windows) and locate regedit.exe.
    Right-click on regedit.exe and select Run as. Uncheck "Protect my computer and data from unauthorized       program activity" and click OK.
    Using Regedit, locate and then click on the following registry key:
    HKeyCurrentUser\Software\Classes
    On the left panel, right-click on the following registry subkey: '.exe'
    Select Delete and then click OK.
    On the left panel, right-click on the following registry subkey:
    'secfile'
    Select Delete and then click OK.
    Close Registry Editor.

Windows Vista and Windows 7:

To open the Task Manager, press CTRL + SHIFT + ESC.
Click File , press CTRL and click New Task (Run…) at the same time. A command prompt opens.

At the command prompt, type notepad, and then press ENTER.

Paste the following text into Notepad:

On the File menu, click Save as.

Select All Files in the Save as type list, and then type Exe.reg in the File name box.

Select Unicode in the Encoding list. Save it and remember the file location.

Return to the Command Prompt window, type REG IMPORT [filepath]Exe.reg, and then press ENTER.

Note [filepath] is a placeholder which is to input your Exe.reg file location (e.g. C:\Exe.reg).

Click Yes, and then click OK in response to the registry prompts.
Log off from your account. Then, log back onto your account.
Note You may have to restart the computer to restore the program icons to their original appearance.
Note After the problem is resolved, delete the Exe.reg file so that it is not mistakenly added back to the registry at a later date.
 Click Yes, and then click OK in response to the registry prompts.

Caution There can be many instances of the registry subkey that is mentioned in step

You must make sure that you are in the appropriate registry subkey before modifying the UpperFilters and the LowerFilters values. To verify that you are in the appropriate registry subkey, make sure that the Default data value is DVD/CD-ROM and the Class data value is CDROM.

3. In the right pane, if you have UpperFilters, right-click UpperFilters, and then click Delete.
4. Click Yes to confirm the removal of the UpperFilters registry entry.
5. In the right pane, if you have LowerFilters, right-click LowerFilters, and then click Delete.
6. Click Yes to confirm the removal of the LowerFilters registry entry.
7. Exit Registry Editor, and then restart the computer.

Creating New Mac OS X account to recover old account.

STEP 1
Restart system and hold “Cmd + S” keys from keyboard until it shows Terminal.

STEP 2
Now type the following commands one after one and press “Enter” button after each command to execute it individually
/sbin/mount -uaw
rm /var/db/.applesetupdone
reboot

STEP 3
“Reboot” command will restart the Mac and you will see the “Welcome wizard” after beautiful welcome messages animation.

Follow all the wizard instructions, until you see the following screen to create new account. Fill all entries and hit Continue button.

This will create a new administrator account and automatically log into it.
Reset old user account password
Now you can reset the password of your old Mac OS X account using this new account, here is the process.

STEP-4
Click on “Apple icon” and then “System Preferences”

Click on “Account”s icon

STEP-5
Here you can see all Mac OS X user accounts are listed, new account “TrickyWays” that we have created and “Susan” the old one that we want to recover.
Click on lock icon to enable prev. account “Susan” and reset its password.

Enter the Password of current user account and hit OK button

STEP-6
Now your prev. account is enabled for changes, select that and hit the “Reset Password” button

Enter “New password”, “Password hint” and hit “Reset Password” button to reset account password.

Now log off and log in to your prev. account “Susan” and you can delete the account “TrickyWays” that we have created to recover prev. account “Susan”.

How to reset the password of the Mac OS using the MAC CD?

Or, put it in the computer and click the Install or Restore icon you see in the disc's main window (after which the computer will start from the disc without you needing to hold C). Or, you can use Startup Manager or the Startup Disk preference pane to select the Install disc.

Choose a language, click the arrow button to continue,
    Mac OS X v10.6 or later: Choose Password Reset from the Utilities menu.
    Mac OS X v10.5 or v10.4: Choose Reset Password from the Utilities menu.
    Mac OS X v10.3: Choose Reset Password from the Installer menu.

    Tip: If you don't see this menu or menu choice, you're probably not started from the disc yet.

    Note: The default keyboard layout is U.S. English while started from the installation disc. If you use a keyboard layout other than U.S English, use the Input menu (the flag icon on the right side of the menu bar) to select the desired layout before typing a new password.
     Select your Mac OS X hard disk volume.
    Select the user name of your original administrator account.

    Important: Do not select "System Administrator (root)". This is actually the root user. You should not confuse it with a normal administrator account.

Enter a new password.
Click Save.
Click the red button in the upper left corner to quit the application.
Choose Quit Mac OS X Installer... from the Mac OS X Installer menu.
Click Restart.
